What is Pelvic Congestion Syndrome (PCS)?
What Causes PCS?
Pelvic congestion syndrome is a vascular abnormality caused by varicose veins that arise from the ovarian veins in the pelvis. Similar to other causes of varicose veins, these dilated pelvic veins are characterized by abnormal valves resulting in chronic progressive dilatation, translating into worsening symptoms.
PCS Symptoms Explained
The most common symptom of PCS is chronic pelvic pain. Additionally, pelvic pressure/ heaviness, pelvic pain with prolonged standing, or dilated varicosities at the vulva or pelvis may be noted.
Risk Factors
Pelvic congestion syndrome is typically diagnosed between ages 20-50. Women with history of multiple childbirths have an increased risk of developing PCS.
When to Consult a Doctor
PCS is typically defined by pelvic pain for at least 6 months in the setting of characteristic pelvic varicose veins identified on imaging. Given the difficulty of diagnosing pelvic congestion syndrome, PCS has historically been a diagnosis of exclusion resulting in unnecessary prolongation of symptoms and discomfort.
Treatment Options for Pelvic Congestion Syndrome
Conservative Management
Pelvic congestion syndrome can sometimes be treated with hormonal medications to reduce blood flow and congestion in the pelvis.
Minimally Invasive PCS Treatments
Embolization of the gonadal veins is the treatment of choice for treating pelvic congestion in order to decrease the size of the veins in the pelvis and therefore improve pelvic discomfort and related symptoms.
